END USER License Agreement

These End User Terms ("Terms") are being presented to you because the organisation where you work has acquired the right to use Headsafe's Nurochek Pro system, including its headset, software, and related services ("Product").

There Terms outline the conditions on which you are permitted to use Nurochek Pro system, including its headset, software, and related services ("Product") in clinical setting.

By proceeding to use the Product, you confirm that you have read and agree to these Terms.

1. Authorized Use

You must only use the Product if you are a licensed healthcare professional or working under the supervision of a licensed healthcare professional.

You must use the Product in accordance with Headsafe "Instructions for Use" and only use the Product for its intended clinical purpose.

2. Clinical Responsibility

The Product is an FDA-cleared medical device (Section510(k) | K243426).

Product is an aid to clinical decision-making.

IT DOES NOT REPLACE YOUR PROFESSIONAL JUDGMENT. YOU REMAIN SOLELY RESPONSIBLE FOR ANY ASSESSMENTS, DIAGNOSES, OR TREATMENTS MADE USING THE PRODUCT.

Any adverse events involving the Product must be reported to Headsafe within 24 hours by email at: support@teamheadsafe.com.

3. Account and Access

You have been allocated user credentials. Please keep these secure.

You are responsible for all activity involving the Product conducted through your account.

4. License and Restrictions

Headsafe grants you a limited, non-transferable license to use the Product.

You must not modify, reverse-engineer, resell, or share the Product or any part of it.

5. Privacy and Data Use

Headsafe complies with HIPAA and other privacy laws.

You are responsible for obtaining patient consent (where the patient is able to give consent) before collecting any data from the patient, including the administering of a Test.

6. Warranty and Support

Headsafe supports the Product. Contact support@team.headsafe.com for support if any technical issues are encountered.

7. Termination

Headsafe may suspend or terminate your access to the product if you breach these Terms.

8. General

These Terms are governed by Florida law.
